The Systems Reality Check, is a practical quarterly assessment framework to surface gaps and pain points across the core areas of an organization. Each operational area is scored across three dimensions: how well it is functioning, the pain it is causing, and whether it is documented. The scores are then plotted on a simple quadrant to identify what needs immediate attention versus what can wait.
